Press Setup button (looks like a piece of paper, under the voicemail button):
Navigate to Menu Item #14 - Factory Reset, and press Select:
Select OK at the Reset Warning...
Commencing Reset and Initialization... (lots of blinking lights...)
If prompted for a password during this, it would be 222222
